Installation Services
• Provide a Plan Document and Trust Agreement
• Provide a Summary Plan Description
• Provide any additional documentation necessary to install the plan

Annual Services
• Provide an annual review consisting of
-Summary of eligible employees in the Plan
-Calculation of benefits and contribution requirements for the Plan
-Calculation of vested benefits or vested account balances for all participants
• Prepare annual report forms (Series 5500 and accompanying schedules)
• Provide loan documents
• Perform all IRS required testing

Optional Services
Any other services requested and agreed to will be provided at an hourly rate, by separate agreement between the parties.
