Text Box: www.retirementplans.com Text Box: AVAILABLE SERVICES Installation Services -Written Plan Document and Trust Agreement -Summary Plan Description for all plan participants -Board Resolution, for use in adopting the Plan -Notice to communicate the new Plan to the employees -Assistance in applying for a Letter of Determination (if applicable) from the IRS -Assistance in applying for any identification numbers required by the IRS Annual Services -An annual review, including, but not limited to: * Summary of Plan Specifications * List of eligible employees in the Plan, with their participation dates * Reconciliation of individual accounts for Defined Contribution Plans * Calculation of vested benefits or vested account balances for all participants * Calculation of benefits and contribution requirements for the Plan * If applicable, non discrimination, top heavy and/or coverage testing results * Statement of assets and liabilities for the Plan -Preparation of annual reporting forms (Series 5500 and accompanying schedules) -Assistance in filing Series 5500 and accompanying schedules with the EFAST2 system -For large filer Plans, assistance with the annual audit -Preparation of the Summary Annual Report -If requested, preparation of forms 1096 and 1099R for distributed participants. Year Round Services -A plan administration expert will be assigned to your Plan and available to answer any questions you have -A secondary contact will be assigned to you in the unlikely event you cannot reach your first contact -Year round processing of loans and distributions for daily valued plans -Evaluation of your Plan design at any time, at no charge. We can make recommendations, and, if desired, provide an amendment for trustee approval. Optional Services -Any other services requested will be provided at an hourly rate, by separate agreement between the parties
